Level One

Parents, teachers and students. Please read through the following 7 details before scrolling down to the survey. Thank You

1.) This survey is not a test. This survey is organized to collect data in order to improve instruction.

2.)
Read through each "Skill and Awareness" section first to familiarize yourself with the entire survey.

3.) Consider each question and complete the survey. Each survey is made up of four parts.


4.) All parts of the survey do not need to be completed at once.

5.) We recommend that parents assist their children with the surveys. We recommend older students ask for assistance if any portion of the survey is unclear.

6.) We recommend that you print this survey packet in order to better record your results as you complete each part of the survey.
